The Homelessness Prevention Case Manager is primarily responsible for assisting individuals to prevent homelessness or assist in exploring alternative safe housing options. Case Managers will network, collaborate and coordinate housing, landlord/tenant communication, connection with supportive services in the community, which may include; employment services, legal services, and health services. The Case Manager will conduct individual sessions, document individual participation, and submit required reporting data.
Education/Experience Requirements: Bachelor’s or Associates degree in health and human services. Case Management experience working with individuals/families from diverse populations.
